Questions practices ask

Answers before you start RPM or CCM.

Will this disrupt our office workflow?
No. The practice does two things: the provider writes the RPM or CCM order, and the patient signs consent at checkout. TOCA handles enrollment, devices, monitoring, documentation, and CPT-ready billing summaries.
Do TOCA care managers make clinical decisions?
No. Care managers work under your name and your protocols — ACC/AHA, ADA, KDIGO, GOLD, and NKF as indicated. Red-flag readings escalate to the prescribing physician. You remain the clinician of record.
What if a claim is denied?
TOCA credits any charge insurance does not reimburse. Net-30 terms mean you collect before you pay. No setup fees and no long-term lock-in.
How fast can a practice go live?
Most practices are live in as little as one day after contract execution. TOCA specialists map your checkout workflow so staff burden stays near zero.
What if a device fails or a patient cannot use it?
Devices are cellular — no Wi-Fi, Bluetooth, or smartphone pairing. TOCA owns technical support, warranty, and replacement. The front desk does not take device calls.
Does TOCA bill insurance for us?
The practice bills. TOCA AI tracks billable time and generates CMS-aligned CPT documentation (99453–99458, 99490 and add-on codes) for your biller, including audit-ready reports.
We already have an RPM vendor. Is switching disruptive?
TOCA runs the transition: patient re-consent, a cellular device swap shipped to the home, and re-enrollment — with no gap in monitoring or billing. Most practices are live in days. A 30-day trial of up to 25 patients lets you compare without a commitment.
Which patients are eligible?
RPM and CCM are reimbursed for Medicare, Medicare Advantage, and many managed Medi-Cal patients with qualifying chronic conditions. TOCA identifies eligible patients from the panel you already have.
Can we use TOCA AI without the care team?
Yes. Software only is free for up to 100 patients. Your staff runs outreach; TOCA AI provides the clinical intelligence layer, time tracking, and CPT-ready documentation. Larger panels and device logistics can be added when you are ready.
Is TOCA AI HIPAA-compliant?
Yes. TOCA AI is operated under Business Associate Agreements. PHI is encrypted in transit and at rest. The assistant speaks only to the care team, never to the patient, and does not invent findings. Access is logged.
