Multimorbidity
CCM 99490, plus 99439 when the month runs long.
RPM for primary care
TOCA follows the conditions primary care already owns: hypertension, COPD, diabetes, and patients living with more than one of them. Cellular devices transmit without Wi-Fi or Bluetooth, and monthly chronic care management runs without adding work to the floor.
Primary care workflow
The kit matches the diagnosis. Hypertension gets a cellular cuff. COPD gets a pulse oximeter. Diabetes gets a glucometer. Multimorbidity gets chronic care management across the whole list.
01
At the visit you already have. Hypertension, COPD, diabetes, or a patient living with more than one of them.
02
A cellular cuff, glucometer, pulse oximeter, or scale — matched to the condition. No Wi-Fi. No Bluetooth. No app.
03
An RN-led team runs the check-in, medication review, and education. TOCA AI keeps the note chart-ready.
04
Escalations come to you when a reading or a symptom needs a clinical decision. You remain the clinician of record.
Clinical use cases
The highest-volume reason to ship a cellular cuff. A month of home systolic and diastolic readings replaces a single lobby number, and the care team reinforces the plan between titrations.
A cellular pulse oximeter for oxygen saturation and pulse. Used when hypoxia, an exacerbation pattern, or recovery after an illness should be visible without asking the patient to join Wi-Fi.
A cellular glucometer for patients who will not complete a Bluetooth setup. Paired with monthly medication review, refill coordination, and hypoglycemia education. You still decide therapy.
Most Medicare patients already qualify for chronic care management. CCM is the coordination layer across pharmacy, specialists, and the next lab. Devices are added only for the numbers you want watched.
Suggested program mix
CCM 99490, plus 99439 when the month runs long.
Add a cellular cuff or glucometer, then RPM 99454 and 99457.
Cellular pulse oximeter, with the same RPM device and management codes.
Hypertension, COPD, diabetes, and patients with more than one chronic condition. The device follows the condition: a cellular blood pressure cuff, a pulse oximeter, a glucometer, or a scale. None of them need Wi-Fi or Bluetooth.
A cellular pulse oximeter sends oxygen saturation and pulse from home. The care team calls when readings fall outside the range you set, and you are looped in when the pattern is clinical rather than a single noisy dip.
No. TOCA uses Tenovi direct-cellular devices. Setup calls about Bluetooth pairing do not happen because there is nothing to pair.
CCM covers monthly coordination for patients with two or more chronic conditions. RPM is added for hypertension, diabetes, COPD, or weight when a device should be in the home. The practice bills both only when the work is distinct.
