About
TOCA Health was founded by nephrologist Dr. Dayan Gandhi. Chronic conditions change between visits, and a fifteen-minute encounter is too short to reconstruct what happened at home. The company exists to make those minutes count — with clinically integrated software, cellular devices, and a care team designed around the practice. Programs serve nephrology, cardiology, primary care, pulmonology, and value-based care organizations.

The founding view
Dr. Gandhi’s clinical career has focused on patients with chronic kidney disease — people whose numbers move at home, who face real barriers to frequent in-person visits, and who are nonetheless trying to stay engaged. That gap between the exam room and daily life is where TOCA was built.
The model is simple on purpose: connected devices for measurement in familiar environments, software that organizes the signal, and care managers who interpret, educate, and escalate. Data alone does not create clarity. Interpretation, communication, and clinical context do.
TOCA remains a physician-led company. We specialize in nephrology, cardiology, primary care, and pulmonology because that is where continuous connection changes the conversation — hypertension, diabetes, CKD, heart failure, and the conditions that travel with them.

Care pathways follow ACC/AHA, ADA, KDIGO, GOLD, and NKF guidelines. Safety you can point to — not a model you cannot inspect.
No long-term contracts. No minimums. Net-30, so you collect first. Unreimbursed charges are credited. Quarterly profitability reviews with a named account specialist.
