The role

You are the primary point of contact between patients and their care. You will own a dedicated panel enrolled in our chronic care programs and build ongoing relationships through monthly outreach, vital-sign monitoring, care-plan management, and thorough documentation.

This is for someone people-oriented, highly organized, and comfortable in a technology-driven environment. You do not need to know everything on day one — motivation to learn, resourcefulness, and commitment to your patients matter most.

What you will do

  • Conduct regular outreach calls — wellbeing, vital-sign trends (blood pressure, glucose, weight, oxygen), and complete documentation.
  • Review remote device data and identify readings that need attention, follow-up, or escalation.
  • Develop and update individualized CCM care plans: lifestyle, medication adherence, and changes in health status.
  • Coordinate with physicians, nurses, and clinical leadership using TOCA escalation protocols.
  • Document interactions, vitals, care-plan changes, and communications in the TOCA platform accurately and on time.
  • Use TOCA’s AI-assisted tools to guide conversations, generate documentation, and stay productive on outreach.
  • Manage follow-up schedules, open tasks, and monthly outreach requirements for your panel.
  • Help patients with device setup, connectivity, and basic troubleshooting.
  • Contribute to a professional, mission-driven team.

What we are looking for

Required

  • 1–2 years in a healthcare, medical office, or care coordination setting
  • Strong telephone skills — warm, clear, and comfortable with patients
  • Excellent written communication for documentation and care plans
  • Organization to manage a large panel without dropped follow-ups
  • Learn new software quickly; troubleshoot basic device or connectivity issues
  • Comfortable with Excel or Google Sheets
  • Reliable, independent, and self-directed
  • High energy, professional, and a genuine interest in helping people

Preferred

  • Bilingual English and Spanish — Spanish-speaking candidates prioritized for immediate hire
  • Certified Medical Assistant (CMA) or equivalent
  • Familiarity with medical practice operations
  • Experience with blood pressure, glucose, or other vitals
  • RPM or CCM program experience
  • Familiarity with generative AI tools and a willingness to use them at work
  • EMR / EHR experience

Our programs

  • Remote Patient Monitoring

    Patients use connected blood pressure monitors, glucometers, pulse oximeters, and scales. Care managers review readings, call when values are outside range, and keep patients engaged.

  • Chronic Care Management

    For patients with two or more chronic conditions, at least 20 minutes of structured, non-face-to-face care each month — care plans, provider coordination, and documented touchpoints.

  • Care team model

    Clinical leadership and provider relationships sit behind every care manager. Escalation protocols mean you are not alone when a situation needs clinical judgment.
